Next, our team carries out a thorough assessment of the prospective resident. This includes an in-depth conversation, an evaluation of individual needs, and a shared understanding of care expectations.
After the assessment, we review all the details and decide whether the home suits the resident. We also confirm how our team will meet their care requirements.
Once everyone agrees on the placement, we request proof of funding—either from the social worker or directly from the family in privately funded cases.
After confirming funding, we schedule an admission date together. To help new residents settle in comfortably, we also encourage families to personalise their space with furniture and familiar belongings.
In the days following admission, our healthcare team starts developing a personalised care plan.
